He is holding a kokosh (Hungarian unyeasted cocoa cake), per the famous story of how he spiritually prepared for the High Holidays: by cutting and preparing sweets for guests. He is surrounded by vines and grapes. as he bestowed unlimited hospitality on itinerant workers who came to the vineyards of Kerestir.
The Kamaya (holy acronym) at the top was given by Reb Shayele to a man being harassed, whose troubles disappeared afterwards.
